For the record...

Here is the FAA report of the New Standard accident on July 26, 2009.

I obtained it by filing a FOIA (Freedom of Information Act) request. Here is a link to the PDF file I created.

Oh....There's some Halloween humor on page 5 of this PDF. The FAA states all of us ended up in a raven!
